  3. Communicate with the primary care physician. Schedule a follow-up appointment with your loved one’s primary care doctor after discharge. 
  4. Understand and watch for side effects. As your loved one’s doctor or pharmacist about possible side effects and be vigilant about watching for them. Report anything out of the ordinary to a doctor.
  5. Utilize available tools. Medication management tools like pill organizers, reminder apps and logs will help your senior stay on track.
